Every athlete at Notus Jr. Sr. H.S. needs a sports physical each and every year.

Who provides them- Dr. Shawn Rudkin
Where- 101 E. Grove Ave. Parma, Id
When- Any Monday, Wednesday, or Friday
Call- 208-722-9999
Cost- 35.00 (Dr. Rudkin donates half of this back to the school district!!!)

You must have a parent / guardian present.
